Czech Republic Visa

The following requests concern visas to be applied by the Consulate General of the Czech Republic in Istanbul.

A valid passport must be valid for at least 3 months after the expiration date of the passport and must have a blank page for the visa; the photograph in the passport must be the same as the person's appearance on the application date.

*Passport Photocopy - photocopy of the passport's picture page, validity period page, identity information page, back page and Schengen visas obtained within the last six months.

*Original application form for each person traveling (including minor children traveling together), legibly and completely filled in and signed by the passport holder. The form must be filled in capital letters, one letter in each box, and nothing must be written outside the boxes.

*1 photo - the size of the photo (3.5 x 4.5 cm) must be on a light background, the face and upper shoulders must be visible, the full image must be 70% of the photo, looking straight ahead. The appearance in the photograph must match the applicant's appearance.

*Travel insurance covering the entire duration of the trip (minimum 30,000 euros, covering sudden illnesses, injuries and return expenses to the country).

In case of a short-term multiple entry visa, a declaration stating that insurance will be provided for subsequent trips after the first trip must be signed.

Travel insurance must be arranged to start at least one day before the start of the trip and end one day after the end of the trip.

For Business Owners

  • Tax Certificate
  • Signature Circular
  • Trade Registry Gazette
  • Activity Certificate
  • Personal bank account (with past certified statement
  • Must have at least 100 TRY per day in the bank account to cover travel expenses) or original credit card statement
  • Flight ticket reservation and hotel reservation or declaration from the person who will provide residence, original and notarized or notarized rental contract must be presented.
  • Consulate officials may request additional documents from the applicant if deemed necessary

For Private Sector Employees

  • Company Tax Certificate
  • Signature Circular
  • Trade Registry Gazette
  • Activity Certificate
  • Payroll
  • SSK Employment Entry Declarations
  • Personal bank account (with past approved statement
  • Must have minimum 100 TRY per day in the bank account to cover travel expenses) or original credit card statement
  • Flight ticket reservation and hotel reservation or declaration from the person who will provide residence must be presented as original and notarized or notarized rental contract.
  • Consulate authorities may request additional documents from the applicant if deemed necessary

For Retirees

  • Photocopy of retirement certificates
  • Personal bank account (with past approved statements
  • A minimum of 100 TRY per day must be in the bank account to cover travel expenses) or original credit card statement
  • A flight ticket reservation and hotel reservation or a declaration from the person who will provide residence and a notary-certified or notary-certified rental contract must be presented.
  • If deemed necessary, consulate officials may request additional documents from the applicant.

Required Documents for Students

  • Student certificate approved by the school
  • A personal bank account (with past approved statements
  • A minimum of 100 TRY per day must be in the bank account to cover travel expenses) or original credit card statement
  • A flight ticket reservation and hotel reservation or a declaration from the person who will provide residence and a notary-certified or notary-certified rental contract must be presented.
  • If deemed necessary, consular officials may request additional documents from the applicant

Required Documents for Housewives

  • Copy of marriage certificate
  • Written guarantee from spouse
  • Spouse's business documents
  • Personal bank account (with past certified statement
  • Must have at least 100 TRY per day in the bank account to cover travel expenses) or original credit card statement
  • Flight ticket reservation and hotel reservation or declaration from the person who will provide residence, original and notarized or notarized rental contract must be presented.
  • If deemed necessary, consular officials may request additional documents from the applicant
